HTMA Client Policy
Scope of Services

Clients understand that Sarah Lomack, RHN, HTMA-P, is not a Registered Dietitian or Medical Doctor.

Clients acknowledge that I am not seeking medical diagnostics, procedures, or treatment through Sarah Lomack's services.

Sarah Lomack’s services are limited to consultations related to Hair Tissue Mineral Analysis (HTMA) and holistic nutrition to support general wellness.

Clients understand that Sarah Lomack does not diagnose, prognose, prescribe remedies, treat, or cure any disease. Her services do not involve any licensed or controlled acts under medical practice regulations.

Clients understand any recommendations provided are not a substitute for medical advice or treatment.

Clients understand that if I require medical attention, it is my responsibility to seek care from a qualified medical professional.

Clients understand that while Sarah Lomack cannot legally advise on diagnosed medical conditions, she may offer nutritional and lifestyle guidance to support my body's wellbeing.
HTMA Testing and Appointment Information

Clients understand that HTMA Test Kits and all appointments are non-refundable.

Clients understand that a one-hour results analysis appointment is required to receive my results.

Clients understand that all HTMA Test Kits and appointments must be paid in full by credit, debit, PayPal, or e-transfer at the time of purchase.

Clients understand that all appointments are conducted virtually.
Email Check-ins and Support

Clients understand that weekly email check-ins will cease if I reschedule an appointment and will resume after the next scheduled appointment occurs.

All “A La Carte” follow-ups follow the same email schedule as the HTMA Bundle.

30 or 45-minute follow-ups that are scheduled more than 6 weeks apart will receive bi-weekly emails for 6-8 weeks. Up to 6 weeks if no follow-up appointment is scheduled. Up to 8 weeks if a follow-up appointment is scheduled.
Clients who purchase an HTMA kit or package will receive weekly email check-ins based on the selected service:
HTMA Bundle:

Up to 6 weeks if a 30- or 45-minute follow-up appointment is scheduled.

Up to 4 weeks if no follow-up appointment is scheduled.
3-Month Package:
-
12–15 weeks, depending on whether the months include 4 or 5 weeks.

All appointments in this package must be used within 5 months of purchase.
6-Month Package:
-
24–30 weeks, depending on whether the months include 4 or 5 weeks.

All appointments in this package must be used within 8 months of purchase
HTMA Retesting Policy

Clients understand that if they purchase a retest, they must book a 60-minute results analysis appointment to review and discuss the new results.

Following an HTMA retest, a new 3-week meal plan will not be provided; however, clients may receive updated wellness recommendations, including lifestyle strategies, supplement adjustments, and additional recipes to continue supporting their health and progress.
Wellness Program Components Following an Initial HTMA Test

Clients understand that they will receive a customized wellness program, which may include:

A 3-week meal plan

Recipes
-
Weekly goals
-
A suggested supplement schedule
-
Additional educational resources deemed beneficial by the practitioner.
Client Responsibilities

Clients understand that the practitioner Sarah Lomack RHN, HTMA-P is not responsible for any misinterpretation and/or application of HTMA test results, or self-supplementation decisions.

Clients understand they will be advised to review any health warnings, allergen information, and dosage instructions on all supplement recommendations.
Pricing Policies

Clients understand that service prices are subject to change at any time without notice.

Clients understand all services are subject to tax regulations (may differ based on the clients' geographical area).
Cancellation, late fees, and Payment Policies
Cancellation Policy:
-
Clients understand that minimum notice of 24 hours before the scheduled appointment time is required to cancel/reschedule an appointment.

Cancellations within 24hrs of purchase may be eligible for a refund. All services are non-refundable beyond 24hrs of purchase.
Rescheduling Policy:

Clients understand that the appointments must be used within the following timeframes:

3-month package: within 5 months of purchase
-
6-month package: within 8 months of purchase
Late Arrivals:
-
Clients understand that if they are late, the appointment time will not be extended, and the session will end at the originally scheduled time.

Clients understand that if they arrive more than 15 minutes late to the appointment, it will be cancelled and a late fee of 100% of the appointment cost may be applied.

Clients understand that if they are more than 15 minutes late, they will be responsible for purchasing an additional appointment if they want to reschedule.
Weekly Email Check-Ins:

Weekly email check-ins are optional; however, not completing them may result in more time during the appointment being spent on updates rather than advancing to next steps and goals.
Acknowledgment

Clients affirm that they are entering this agreement voluntarily and not under duress of any kind.

Clients understand that they have been offered the opportunity to seek independent advice and to ask Sarah Lomack RHN, HTMA-P any further questions before continuing with services.

Clients acknowledge that all services and support provided are designed to empower clients to take an active role in their own holistic health journey.
